Description:

Our client is seeking a full-time, permanent Licensed Practical Nurse to provide nursing support to clients receiving medically supervised withdrawal and stabilization services in a residential setting. The LPN will assist medical staff with client assessments, medication administration, monitoring, documentation, and coordination of medical care.

Responsibilities Include:

• Participate in screening clients’ medical needs and gathering admission information.
• Observe and monitor clients’ physical, mental, and emotional status.
• Work closely with medical and clinical staff to meet individual client treatment needs.
• Provide health-related education and group counseling sessions.
• Monitor and document client medication administration.
• Take and document client vital signs as ordered by the medical provider.
• Collect and submit urine drug screens to the appropriate laboratory.
• Document laboratory results in client medical records in a timely manner.
• Monitor clients for medical needs and report findings to medical providers.
• Maintain accurate records, reports, and shift-to-shift communication.
• Participate in weekly case conferences, staff meetings, and required trainings.
• Report incidents involving client medical occurrences.
• Contact emergency services, including ambulance, fire department, police, or CDPC, as needed.
• Maintain client confidentiality and support a safe and secure residential environment.
• Work in other residential buildings as needed.

Qualifications:

• Current New York State LPN or RN license required.
• Valid New York State driver’s license required.
• Current CPR and First Aid certification required.
• Two years of nursing experience preferred.
• Experience working in a NYS OASAS residential facility preferred.
• Knowledge of substance use disorder treatment and residential care preferred.
• Strong communication, documentation, and active listening skills.
• Basic computer skills, including electronic health records, email, and Microsoft Office.
• Ability to work effectively with clients, medical providers, and multidisciplinary staff.
